2007-11-21 Worcester man killed in forklift accident
A WORCESTERSHIRE father has been killed in a freak forklift truck accident.
Adam Millichip, aged 27, from Tenbury, died after he became trapped between wooden pallets and a forklift truck in Hampshire.
Paramedics rushed the dad-of-one to hospital, but there was nothing doctors could do to save him.
A joint investigation has now been launched by Hampshire Police and the Health and Safety Executive to establish how the accident happened.
An emotional statement from Mr Millichip's family read: "Adam leaves a devastated five-year-old son Luke, girlfriend Corrie-Ann, parents Sue and David, brother Stuart and sister Amanda.
"We would ask that anyone with information regarding this incident comes forward to help the police with their investigation."
"Mr Millichip was trapped between the pallets and the truck."
He was taken to Southampton General Hospital, but later died.
Sgt Glen Barham, of the Road Death Investigation Team, said: "This is an extremely tragic incident and we are very keen to hear from anyone who witnessed the incident or the events leading up to it.
"This is a joint investigation and we are working closely with Her Majesty's Inspector of Health and Safety to determine exactly what happened."
